Compassionate End-of-Life Care

Our Philosophy

To provide compassionate, supportive palliative care to those individuals diagnosed with a life-threatening illness, where cure is no longer the goal but quality of life is always the focus.

The Hospice Team

The hospice team is an interdisciplinary group of professionals trained in palliative care. We are medically-directed, nurse-coordinated, and patient-focused.

Physician

The physician manages the medical components of your care. This may be the hospice pvhysician or your own.

Hospice RN Coordinator

A registered nurse will manage your case. The RN will focus on any quality of life issues as defined by you or your family. They will visit you in your residence at home, hospital, or another facility intermittently to assess your needs and coordinate your care.

Hospice Nurses

RNs and LVNs may make occasional visits based on the care plan developed by your Hospice RN Coordinator. Hospice Nurses visit on a schedule set by the Hospice RN Coordinator. Each visit lasts one to one-and-a-half-hours.

Counselor

We can arrange for counseling for you and/or your family for support, bereavement, dietary counseling or more, depending on your needs.

Hospice Aide

Our Hospice Aides are certified nurse’s aides with additional training in hospice care. Your Hospice Aides will provide assistance with your activities of daily living, such as bathing, hygiene, dressing, and more. They will also change linens, provide meal assistance, and spend time with you for emotional support. They usually visit five days a week for about an hour.

Hospice Volunteer

We are fortunate to have volunteers who donate their time and talents. The heart of hospice care, our volunteers are dedicated to supporting hospice patients with our mission of “To Care Always” at the forefront.

Spiritual Care

Your hospice team will provide a list of clergy dedicated to spiritual counseling and support. If you wish, we will contact them and coordinate visits for you. We believe that all human beings have a spiritual core and our goal is to respect the unique spirituality of every individual as they define it.

Other Services

We strive to meet all of your needs to maintain or improve your quality of life. With that in mind, Physical Therapy, Occupational Therapy, and Speech Therapy are all available to you. A registered nurse is on on hand 24 hours a day, seven days a week for On-Call Services. You will be provided with a telephone number to call and the nurse will determine whether a visit by a member of your hospice team is necessary.
